LDS hospital is a 220-bed facility located in Salt Lake City, Utah. It is owned and operated by Intermountain Health Care, a non-profit health care organization, serving the needs of Utah and Southern Idaho residents. LDS Hospital provides Critical Care, Joint Center, General Orthopedics, Musculoskeletal, Medicine, Oncology, Bone Marrow Transplant, Surgical, Women and Children's, Diagnostic Cardiology, Fitness Institute and Behavior Health services to the community it serves.



This position provides outpatient anticoagulation monitoring services to orthopedic patients and clinical pharmacy services to maternity/Labor & Delivery. This is an excellent opportunity for a clinical pharmacist who has a desire to develop a career in a unique setting with a team led by excellent physicians, nurses and pharmacists.

Clinical pharmacists at LDS Hospital are involved in therapeutic drug monitoring, medication reconciliation, mentoring interns/students, TPN ordering, reviewing patient histories, medication reconciliation, reviewing physician orders, consulting with physicians, implementing/promoting medication management principles, providing formal/informal education, supervising technicians and patient counseling. The Pharmacy Department utilizes Robotics, Automated Dispensing Machines and Bar Code Technology to support a safe medication distribution system. Staff provides these services in accordance with governmental regulatory agencies, corporate guidelines, policies and procedures. Skills / Requirements

Must have a license as a Pharmacist in the state of Utah and be able to dispense controlled substances.

Bachelor's degree in Pharmacy or a PharmD degree from an accredited school of Pharmacy.

Residency experience and certification PharmacoTherapy (BCPS) preferred; experience and training in a hospital pharmacy environment are preferred.

Demonstrated effective communication and interpersonal relations skills are required.
