Completion of Accredited Physician Assistant educational program.
Current Utah Physician Assistant license with ability to be credentialed through Intermountain Healthcare.
National certification as a Physician Assistant.
Current Utah Controlled Substance License II through V.
Current Federal DEA Certificate II through V.
Current BLS certification.
Previous experience as a Physician Assistant preferred.
